Disability Care
Dignified Disability Care has established a very high standard for person-centric service, focusing on attending to the individual needs of our participants while also giving them the care and assistance they desire and keeping their expectations at the forefront of all decision-making. Our support managers and support workers’ main focus is ensuring that each person succeeds in obtaining their independence, desires, and freedoms. Our services are provided with the utmost respect and dignity, maintaining the value and worth of our clients. We assist NDIS Participants in achieving their objectives by:
- Housing accommodation options
- Respite care
- Community participation
- In Home Care
- In Home Care – Personal Care
- Household duties including gardening
- Meal preparation
- Simply going for a coffee or anything else you may need
Housing Accommodation Options
Home isn’t a place; it’s an emotion, according to Dignified Disability Care, so your house should reflect who you are and be filled with the things you love.
Dignified can help you determine what style of housing accommodation options suit your needs. We can assist you in understanding your options, whether it be Supported Independent Living, Specialised Disability Accommodation, Short Term Accommodation, or even how we can find the right accommodation in the rental, Department of Housing space.
Dignified’s support teams are made up of individuals who are carefully selected and highly-skilled, delivering professional and dignified support. We provide services for individuals with funds for Supported Independent Living (SIL) in their NDIS plan who require 24/7 supports will have supports that include;
- frequently available around-the-clock supports
- fostering personal development in a secure atmosphere with the aim of increasing your level of independence at home
- establishing connections with your neighbourhood.
For persons of all different levels and abilities wishing to live in a shared home environment with friends who have similar interests and disability assistance needs.
Our dedicated and compassionate team is here to support you and will provide both in-home and community supports whilst in our care. Our staff will assist you in organising your daily menus taking into consideration all your dietary needs.
Individual Living Options (ILO) are living arrangements that focus on the individual. They consist of choices for living alone, co-residency, host arrangements, and cohabitation options. ILO is an alternative to group home services.
What is the difference between SIL & ILO?
“ILO (Individualised Living Options) include at least six hours of support per day and can be up to 24 hours of daily support. SIL (Supported Independent Living) are living arrangements with rostered support staff and 24/7 paid support.” NDIS
Dignified Disability Care also support clients that have very high needs or extreme functional impairment and require specialised modifications to the home to accommodate the clients needs. These homes are referred to as Specialised Disability Accommodation (SDA). This style of housing needs to have SDA funding in your NDIS Plan.
For those people who essentially need time to spend away from your typical living arrangements, Dignified Disability Care offers Short Term Accommodation (STA), also known as Respite. STA supports may be required for only one night away or up to several weeks depending on the needs.
Community Participation
Being active and engaging with your community is important for well-being. Social and community participation is about meeting people with similar interests and joining in social and community activities. It means going where others go, doing what others do, being with other people and choosing activities you enjoy. Social and community participation can also mean different things to different people. At Dignified Disability Care we can provide you with supports to help you achieve your goals and maintain a healthy lifestyle including:
- connecting you with your community
- participating in sports or hobbies
- socialising or other group activities
- attending TAFE or other educational needs
Dignified Disability Care also is proud of our monthly activity calendar which offers a variety of opportunities to meet with others socially and enjoy expanding lifestyle skills to assist you reaching your goals.

In-Home Care – Lifestyle Assistance Services
From the moment we meet, we are committed to delivering quality care allowing you to relax and let life continue to be enjoyed in comfort through every season of life.
We will be there to support you and your family through challenges that may arise. If your needs change, we will listen and understand. We will ensure you are treated with the same respect, honour and dignity that we would show to our own family.
Our Lifestyle Assistance Services are available to you from just 4 hours per week up to 24-hour care.
Live better with our Lifestyle Assistants who will help you with….
- Daily personal and hygiene care
- Meal preparation and light household duties
- Coffee/lunch outings, personal/food shopping
- Garden maintenance, technology assistance
- Community access to your Health & Well-Being activities including medical appointments
- Respite Care
- Specialist Support (eg: wound and stoma care)
Dignified Lifestyle Assistance Services provides affordable in-home services for self-funded retirees, clients with home care packages or patients in post-hospital recovery. Our specialist staff and carers are highly qualified in aged care supports.
Fee Structure
Trilogy Care Package Management Fee covers the cost of managing your Home Care Package including invoicing, statement, claiming and reporting through government systems, compliance and quality activities required as determined by the government.
Services Provided Include: light house duties, laundry, meal preparation, personal cares, garden maintenance, technology assistance, community access; medical appointments, food/personal shopping, coffee/lunch outings, movies/concerts/sporting events.
In-kind services – Care Management to provide you with more hours of care and less money spent on fees we will assist you with your care management complimentary. This includes:
- Sourcing equipment, transport requirement, meal plan management, multicultural services
- Sourcing and assisting with GP’s, Medical Allied Health e.g. podiatry, occupational health therapist, dietician.
* Hourly rate is based on Monday-Friday 06:00am – 08:00pm. Respite, overnight, weekend and public holiday rates provided upon application.
The Process
Once you contact us, a member of our Care Management team will arrange to meet with you and your family for a free consultation where we will listen to you to understand your specific needs and discuss your options.
You will then be introduced to your carefully selected Lifestyle Assistant and our in-home services will begin!
Our dedicated Support Management team will stay in regular communication with you to ensure you are satisfied with your team and the care you are receiving.
